[Council] JEP-0004 errata

A colleague of Ralph Meijer's pointed out to him several small errors 
in JEP-0004 (Data Forms), to wit:

1. The syntax description for the <reported/> element in Section 3.4 is 
probably incorrect when it includes a <value/> child in the <field/> 
elements within a <reported/> block. I think these should be empty 
<field/> elements and have provisionally corrected the syntax example 
and text accordingly.

2. The <reported/> block in Example 8 is wrong, since it contain four 
fields but the <item/> elements contain only two fields, and Section 
3.4 states "Each <item/> element defines one item in the result set, 
and MUST contain each field specified in the <reported/> element 
(although the XML character data of the <value/> element MAY be null)." 
I have provisionally addressed this by removing from the <reported/> 
block both the "description" and "category" fields in Example 8.

The Council will need to approve these changes in order to publish a 
revised version 2.6 of JEP-0004. I have cc'd Ryan Eatmon (lead author 
of JEP-0004) to make sure that these changes meet with his approval as 
well.
