In general, it seems that we need to define detailed error flows for the
core XMPP stuff, as we did recently in JEP-0060. I'm not sure that we'll
put all that in rfc3920bis and rfc3921bis, but it would be helpful to
define it somewhere, I think. I'll try to work up an initial version soon.
