I don't really know when and why it happens.
When I was running my server on jabberd14 I several times got reports,
that user lost his roster. After investigating, only one connected these
cases - the client was Psi. Even I had lost my roster one time when I
was using Psi. But I do not know when exactly did that happen.
Once I switched to jabberd2, the case of lost roster never appeared
again.
