IMO, it should be MUST. I want to be certain that PEP in its simplest
form (i.e. without custom configuration) behaves the same everywhere,
and the default access model is probably the most important part.

Can this also be a 'the server MUST create a node upon first publish if
it the node does not exist yet?'. Because otherwise there's no way to
know for a simple client (that does not support node customizations)
whether or not he has to create his nodes. And node creation is a
tedious job from the client's point of view.
