[Standards] XEP-0163, v. 1.1pre1

I've taken a first pass at incorporating the publish+configure 
modifications we have discussed on the list:

I also re-organized the document somewhat for better readability and 
tighter protocol definition. The changelog is as follows:

Defined atomic publish+configure action for use on first publish; added 
friendly but non-normative How It Works section to introduction; 
tightened up definition of normative sections; explicitly defined 
auto-create and publish-and-configure features.
