[Standards] XEP-0045 v. 1.23pre4

I've commented out all text and examples about the outsider role from
XEP-0045 (Multi-User Chat) pending further review. However, I think it
would be good to publish 1.23 without the outsider role, since there are
some helpful corrections in the SVN version.
