[Standards] error 'code' attribute

Someone pinged me via IM just now about the 'code' attribute for the
error element. This was retained in the schemas defined in RFC 3921 for
backward compatibility, but I think it can probably be removed now (it
has been almost 3 years since RFCs 3920 and 3921 were published). Also
it's probably time to deprecate XEP-0086.
