When I woke up this morning I thought of an alternate way to do this:
1. Requesting entity sends presence to responding entity and includes
entity capabilities data, including a namespace of
'http://jabber.org/protocol/disco#items+notify' (indicating that it
wishes to receive notifications related to disco#items).
2. Requesting entity sends standard disco#items request (without
inclusing pubsub <subscribe/> since the caps data resulted in an
implicit, short-lived subscription).
3. Responding entity sends standard disco#items response.
4. Responding entity sends subsequent notifications.
5. When requesting entity goes offline, responding entity receives
presence unavailable and terminates short-lived subscription.
