[Standards] XEP-0065 v1.8rc1

While flying to Brussels for XMPP Summit #8, I wrote up some edits to
XEP-0065 but never got a chance to type them in. I've done that now. The
result is here:

The diff is quite large because I moved things around, deleted some
sections, added new sections, etc. Much of this is just a content
refactoring to make the spec more readable. However, there are some
substantive changes too.

I probably won't have time to look at this again for 2 weeks.
