[Foundation] end-user survey

I've started working on an end-user survey so we can gather some
information about Jabber users. Here is a draft:

I copied some of the questions from the OpenOffice user survey but
drastically cut the number of questions so it doesn't take too long to
complete. Is there anything else you think we should ask about?
