Sorry about the delayed reply.

I think it would be helpful for us to provide a form of membership 
application since that is mentioned in the bylaws:

"to be eligible for membership, a person, corporation, organization, or 
other entity must complete a written membership application in such form 
as shall be adopted by the Board of Directors from time to time."

I was surprised to see (or remember) that the bylaws allow membership by 
entities other than natural persons. I will propose separately that we 
modify the bylaws to allow only natural persons to be members. This will 
simplify the Form of Membership Application a bit.
