The affiliation state chart is still inconsistent with the rest of the
JEP. The admin row needs to have all instances of "Admin or" struck out.
I'll look through the rest of it tonight or tomorrow and send my nit-picks.

Once I'm happy w/ the state of 0045 I suppose I can write up another JEP
to fill in any holes. Thinking about some of the things that I changed
in MUC+ might be fit for an informational JEP or for the implementation
notes of 0045. We'll see what I turn out.
