Indeed.

It seems that adding PubSub to the mix won't solve the problem because 
PubSub has the same problem.  And if a new protocol were developed to 
get around the problem in PubSub, then why not use the same protocol for 
MUC, and for that matter any other multicast that needs to happen?

An implementation curiosity, what happens with presence between servers? 
  Does the server send the user's presence to another server once and 
let the server distribute that to all contacts on that one server?
